The Care Co-ordination team

What do they do and how can they be use to you when looking after a loved one who is elderly and showing signs of forgetfulness when you are trying to hold down a job and deal with the rest of your family? I am not sure how much of this is common knowledge but the Care Co-ordination team are an NHS Service in partnership with your GP’s and community health services helping the elderly, stay healthy and independent at home and who may also be at risk of falling, loneliness, and housing issues as well as .
